2026 standard Part B cost

The standard Part B premium is $202.90 per month in 2026. The annual Part B deductible is $283. IRMAA does not replace the standard premium: beneficiaries in an income-related tier pay the standard premium plus the Part B adjustment shown below.

Part D plan premiums vary. The last column is only the income-related amount added to the beneficiary's plan premium. Social Security generally deducts the Part D adjustment from benefits or Medicare bills it directly.

Married filing separately after living together

A different compressed table applies when a beneficiary filed married filing separately and lived with a spouse during part of the tax year. MAGI of $109,000 or less receives the standard premium. MAGI above $109,000 and below $391,000 receives a $446.30 Part B adjustment and a $83.30 Part D adjustment. MAGI of $391,000 or more receives the highest adjustments.

Which income year applies

The current SSA-44 form states that 2026 adjustments generally use MAGI from the 2024 federal tax return, or 2023 when 2024 information was unavailable. The notice from Social Security is the controlling place to confirm which return it actually used.

A Part D amount shown here is not the full drug-plan premium. The IRMAA calculator also excludes the plan-specific premium.
